There's usually two ways, they're welded on or they use metal brackets.
Unless you're a certified welder I wouldn't touch the frame with a welder especially on a critically stressed area on a frame. I'm a manufacturing engineer and I've even welded little brackets on the frames of my cheapy scooters.
Once you weld on the frame the weld area turns brittle and prone to rusting and cracking. I've tried various welding rods and it's just the welding heat affected zone turns brittle and rust prone, likely due to carbon migration in the steel but it's just not worth it to weld critical parts to these frames.
So the only other way is to use bolt mounted hardware. You have to get creative at this point though because depending on the scooter type you might only have a single tube frame in which case it's gonna be near impossible to do a sidecar using just bolts. With a double tube frame it's a lot easier.
But this is one of those things where if you knew you wouldn't have to ask. If you don't you probably shouldn't attempt it yourself.

As for welding I was a pipe fitter for 30 years. I wouldn't try to welding on a Chinese scooter. Take a look at the picture, that scooter is the same as mine, but I don't like the 3000 to 5000 price tag. I just thought that some one on here would know how to do it Attachments:

A trike would be better, especially if the sidecar is not mounted precisely, the tires will die early due to misalignment

A nice sidecar like that Inder is going to cost more than the scooter. They do attach *fairly* easily. Two mounting points. Some mount with three. Just remember that a sidecar on any two wheeled vehicle is going to SIGNIFICANTLY affect the riding characteristics. Many people just have no idea how to properly turn or brake with one. I don't think most scooters have the stability or power to PROPERLY handle one. That Edge body style injim posted looks nice, but I bet it gets really squirrely on a fast brake.
